Attention: An Important Notice about a Telephone Scam

Granite VNA has learned of a scam involving a person or persons who have "spoofed" our main phone number at (603) 224-4093.
 
Spoofing is when a cybercriminal poses as a trusted entity to get something beneficial to them and detrimental to a community member. These cybercriminals are calling community members, and are posing as agency employees attempting to sell Medicare Advantage products. The caller claims they are from Concord VNA, one of our agency names before our merger in April 2021.
 
Granite VNA is not involved with these fraudulent practices and does not make solicitations. Please be aware of this scam, and if you receive solicitations from anyone posing as an agency employee who appears remotely suspicious, please contact us immediately.
 
Please do not respond to these phone calls and do not provide the caller with any personal or financial information.
 

Financial Assistance

Financial Assistance applications are provided to any individual seeking Granite VNA services who are uninsured, underinsured, or otherwise unable to pay the published fee for service. If you have already received financial assistance through Concord Hospital or the NH Health Access Network, we honor that reward.

The current Health & Human Services Poverty Guidelines are used as a guide in the evaluation process for fee scaling. Adjustments may be made according to the current annual income level of the applicant.

Applying for Assistance

The completed application must be submitted to Utilization Review for review to be considered for fee scaling. Any applicant with total assets greater than $100,000 are not considered for financial assistance. Cash assets of $5,000 or more must be applied toward the Granite VNA medical bills before any financial assistance will be provided.

Applicants for Financial Assistance are notified of the decision in writing within 24 hours of receipt of the completed application. All applications received after 4 p.m. on Friday until Monday morning are responded to on Monday. All applicants for Financial Assistance or their representatives are encouraged to contact the Chief Financial Officer at (603) 224-4093, ext. 85669 if they want to discuss the application decision.
