Advanced Illness Management Program Update

Carol Sackett, a certified hospice and palliative care nurse, is our new Advanced Illness Management (AIM) Program Coordinator and Davis Richmond, a licensed independent clinical social worker, is the new AIM Medical Social Worker. Carol is also board certified in pain management.

Carol is available to provide care to homecare patients with advanced stages of a serious illness. She provides expertise in symptom management visits and engages patients and families in goals of care conversations. Carol will also be an ambassador for all of our services by offering education, in-services, and guidance on how patients can access their home care and hospice benefits. 

Davis will work collaboratively with Carol, patients, caregivers, and other healthcare team members to ensure a smooth transition across all care settings by coordinating clinical care and services. He will offer support and facilitate the re-engagement of homecare services, initiation of hospice services, new referrals to Capital Region Palliative Care, and re-engagement with the palliative care interdisciplinary team.
