One-on-One Patient Care

From Bedford to Tamworth, Washington to Middleton, and all Capital Region, central NH and Lakes Region cities and towns in between, our clinicians visit patients in their homes where they are most comfortable. Community-based means we care for our friends and neighbors, and our person-centered approach allows you to connect with patients in a more personal way.

Home Health Nurse Residency Program

The Nurse Residency Program is a year-long transition to practice program that serves as a bridge to support and enhance the professional development of new graduate nurses entering home care practice. Working with a trained preceptor, new graduate nurses gain valuable experience from:
  • An in-depth understanding of your role through exposure to everyday hands-on practice
  • Managing complex patients with skilled nursing care
  • Development of critical thinking skills
  • Managing complex patients through professional nurturance and mentorship
  • Ongoing education and training
