Similar to years past, providing community benefits remains at the heart of Granite VNA’s not-for-profit, community-based mission.
Total Community Benefit
In fiscal year (FY) 2023 (Oct. 2022-Sept. 2023), Granite VNA invested $5,220,982 of its annual budget to deliver community benefit programs and services to meet the evolving needs of people of all ages in our communities. Community benefits increase access to care and help the most vulnerable population of our communities receive the care needed to self-manage their condition, improve their quality of life, and maintain their independence.
Community Health Education
Community Impact: $137,590
Granite VNA partners with senior centers, churches, public libraries and others to provide evidence-based and community wellness education programming in the 82 communities we serve. Our team offers a wide range of in person and virtual programs on topics such as aging well, advance care planning, caregiving, and healthy lifestyles.
Charity Care
Community Impact: $120,700
Granite VNA provides charity care to patients who are deemed unable to pay based on formal financial assistance policies established by the agency.
Baby's First Homecoming
Community Impact: $56,729
Granite VNA provides in-home nursing visits for new mothers and their infants to safely transition from Concord Hospital. During the visit, registered nurses complete a newborn and maternal assessment and provide education on caring for the infant and postpartum care.
Bereavement
Community Impact: $91,212
Bereavement and grief support are for families, friends, and anyone in the community who has had loved ones pass away.
Hospice House
Community Impact: $2,036,658
Hospice House is a warm and inviting home where people who are terminally ill receive 24/7 personalized interdisciplinary care, guidance, and support from caring and compassionate hospice team members. The facility opened in 1994 and was the first Hospice House in the state.
Nutritionist
Community Impact: $105,217
Our Nutritionist assesses, plans, and provides services to home care patients who are at risk of malnutrition and/or in need of the services of a registered dietitian. The Nutritionist also partners with the Community Wellness team to support education programming and the work of our Community Health Educators.
Foot Care Clinics
Community Impact: $90,078
Registered nurses provide foot care, blood pressure screenings and more for older adults in clinics throughout our service area to meet community need.
Leadership Development; Training for Community Members
Community Impact: $238,898
Granite VNA helps develop leaders in the communities we serve and many team members provide volunteer training and education, and participate on local boards.
