While we provide highly skilled health care services to people in their homes and at Hospice House, an important part of our mission is community wellness.
We offer a comprehensive array of programs on topics such as aging, healthy lifestyles, advance care planning, grief and bereavement, caregiving, and more throughout the communities we serve.

Our experienced team partners with community members to help them meet their goals. There is no fee to attend the programs listed below.

Falls are common as people age, but it does not have to be that way. Learn simple ways to reduce your risk of falling during this program. “A Matter of Balance” helps older adults learn to reduce their fear of falling and gain self-confidence to help them remain active. This eight-week nationwide program from MaineHealth is for older adults who want to:
- View falls and fear of falling as controllable
- Set realistic goals for increasing activity
- Change their environment to reduce fall risk factors
- Promote exercise to increase strength and balance

Whether in person or online, Caregiver Connection programs offer family caregivers a valuable opportunity to connect with one another and engage with a team of professionals. These programs provide a safe, supportive space where caregivers can share experiences, find encouragement, and navigate challenges together.

Healthy Families America-NH is a home visiting service available starting when you find out you are pregnant through your child’s 3rd birthday. It is non-judgmental support–when you want it–where you want. Healthy Families America-NH is free, convenient, voluntary and flexible.

Healthy Steps for Older Adults is an evidence-based fall prevention program for adults ages 60 and over. Through activities, assessments and discussions, participants learn tools and tips on how to reduce falls, one of the leading causes of the loss of independence and hospitalizations.
This comprehensive two-hour, two session program focuses on managing fall risk and staying healthy, active, and independent.
Developed by the Pennsylvania Department of Aging in 2005, Healthy Steps has been shown to decrease the incidence of falls, frequency of self-reported hospital and emergency department use, and costs associated with hospital and ED use.

Are you a family caregiver? “Powerful Tools for Caregivers” offers valuable tips to take care of yourself while tending to your loved one. This six-week program helps family caregivers learn to take care of themselves by:
- Reducing personal stress
- Changing negative self-talk
- Communicating more effectively
- Handling different feelings
- Making challenging decisions

The Memory Cafe is a joyful and creative social experience for people with memory loss and their caregivers to gather with others in a positive and engaging environment. Cafes are offered in the Concord region.
