Granite VNA, New Hampshire’s largest non-profit home health and hospice agency has hired Tammy Martel, RN, as Hospice Community Manager. Martel served as a community hospice nurse in the past and was hired from Presidential Oaks in Concord.

In Martel’s new role, she will support the clinical operations of the Community Hospice Program through organizing the hospice community team, advising in patient goal setting, and supervising the home hospice nursing team.

“Tammy’s leadership, communication, planning and clinical skills make her an excellent fit for this role,” said Granite VNA Senior Director of Patient Care Services Cindy Bergeron. “Her dedication to our patients and her peers is evident in everything she does.”

Since leaving Granite VNA in 2022, Martel has served as a nursing director and short-term rehabilitation unit manager at Presidential Oaks in Concord. Martel holds an Associate of Science in health science from New Hampshire Technical Institute.
