Granite VNA Strengthens Home Care Team with New Home Care Management Roles

Home health and hospice agency Granite VNA recently promoted healthcare specialists Kris Nesheim, OT, Letitia Borelli, PT, DPT, and Kristen Hayes, to home care managers. In this newly created role, each will oversee the clinical business operations and patient care services for the agency’s home care services within a specific region of the agency’s service area. They will also mentor and supervise patient care teams, comprising certified nursing assistants, nurses, therapists and social workers.
The addition of the home care management role adds depth to Granite VNA’s home care team and further enhances the agency’s high quality, patient-centered care. By enhancing communication and coordination within and between care teams, the home care managers will play a critical role in ensuring that patients and their families receive seamless care.
Nesheim will help oversee the home care team in the Capital region; Borelli will help oversee the home care team in the Lakes region; and Hayes will help oversee the home care team in the Manchester area.

Kris Nesheim, OT
Nesheim brings 26 years of experience in a variety of clinical and educational settings, including 17 years in home health. Prior to joining Granite VNA in 2015, she served as an outpatient and acute care occupational therapist for Elliot Health System, as a traveling occupational therapist for Soliant Health, and as an occupational therapist for Visiting Nurse Association of Manchester & Southern New Hampshire and other organizations. Nesheim currently serves as an instructor at the Norton School of Lymphatic Therapy.
Nesheim holds a Bachelor of Science degree in occupational therapy from the University of Florida and an Associate of Arts degree from Santa Fe Community College.
Letitia Borelli, PT, DPT
Borelli brings 15 years of experience as a physical therapist in a variety of healthcare settings to her new role, including Spaulding Academy and Family Services, Home Health and Hospice Care in Merrimack, and Birth to Three of West Virginia.
Borelli holds a Doctor of Physical Therapy degree from Massachusetts General Hospital Institute of Health Professionals and a Bachelor of Science degree in exercise physiology from the University of Massachusetts, Lowell.
Kristen Hayes
Prior to joining Granite VNA in 2006, Hayes served as a medical assistant for Family Tree Healthcare in Hopkinton, and as a licensed nursing assistant (LNA) for HealthSouth Rehabilitation Hospital in Concord, where she supported clinicians with patient care and helped patients and their families prepare for the transition home.
Hayes holds a Bachelor of Science degree in healthcare administration from New England College.
