Advanced Illness Management

Advanced Illness Management (AIM) is a specialized program to support patients in all of the communities we serve, who are coping with difficult to manage medical situations, and new or worsening health symptoms.

Goals of Care

By meeting with patients and families in their homes, or other healthcare facilities, our AIM team learns about each individual, their medical challenges, and their life goals. Using this knowledge, our AIM team then provides information about healthcare options that assist patients in attaining their goals of care.

Services Provided

The AIM team consists of a registered nurse and medical social worker. Our team provides patients and their families the following services:

  • Collaborative consultation regarding clinical and social service needs, facilitating goals of care discussions, and healthcare options
  • Information about Granite VNA services and programs to best meet individual healthcare needs and goals
  • Discussion, review, and completion of advance directives
  • Emotional support for caregivers in coping with the decline of a loved one
  • Education regarding what is offered through the Hospice benefit
  • Facilitation of a smooth transition to Hospice services when appropriate
